"Graded and sourced" is only a real claim if it's checkable over time. Two things are logged here: every time an entry's evidence grade moves, and every time we got a fact wrong and fixed it. A site that publishes doses should be willing to publish its own mistakes.
Where a factual claim on a page was wrong and has been fixed — a dose, an interaction, a mechanism, a citation that did not say what we said it said. Wording and typo fixes are not listed: they would bury the entries that matter.
