"Adrenal fatigue", and what it usually is instead

The tiredness is real. The label is not, and chasing it can mean missing what is actually wrong.
Stage by stage
- Where it came from (A 1998 lay construct): Coined by a naturopath, proposing that sustained stress exhausts adrenal cortisol output in stages. The stages were asserted, never derived from clinical data.
- When it was tested (58 studies, no support): A 2016 systematic review found no consistent link between prolonged stress and impaired adrenal function. No endocrinology body recognises the diagnosis.
- What it usually is (A testable differential): Thyroid, iron, B12, sleep apnoea, coeliac, diabetes, depression, insufficient sleep. Most of it is cheap to test for, and several worsen while untreated.
- The real disease (Adrenal insufficiency): Addison’s is rare, confirmed by an ACTH stimulation test, and treated with prescribed hormone replacement. Untreated crisis is life-threatening.
What the evidence supports
- Prolonged stress exhausts the adrenal glands, causing "adrenal fatigue" — insufficient evidence. A systematic review of 58 studies found no consistent supporting evidence, and no endocrinology body recognises the diagnosis. This is a well-tested negative rather than an open question.
- Salivary cortisol panels can diagnose it — insufficient evidence. Not validated for this purpose, and heavily confounded by sleep timing, shift work, and recent food.
- For context: adrenal insufficiency (Addison’s disease) is a real and distinct condition requiring treatment — strong evidence. Unambiguous — and importantly, a different thing. Diagnosed by ACTH stimulation testing and treated with hormone replacement.
- For context: fatigue has common causes that are cheap to test for — strong evidence. Thyroid disease, iron and B12 deficiency, sleep apnoea, coeliac disease and diabetes are all well established and all testable.
- Evidence that it causes Harm from "adrenal support" products — moderate evidence. Documented cases of products containing animal adrenal tissue, pharmacologically active liquorice, and in some cases undeclared glucocorticoids. The larger harm is usually the delayed diagnosis.
Timing
Nothing here is a supplement recommendation. If you have been exhausted for weeks, the highest-value action is a GP appointment and a standard blood panel.
Time to effect
- 1–2 wks: Basic bloods can exclude most common causes.
- Weeks–months: Improvement once an actual cause is treated.
Common myth
“My adrenals are burnt out from stress — I’m in stage 3.” In reality: The stage model comes from a 1998 naturopathy book and has never been validated. When the underlying claim was systematically reviewed across 58 studies, no consistent evidence of stress-induced adrenal exhaustion was found. The fatigue is real; this explanation for it is not.
