Stage by stage
- Intake (Root extract capsule): Active withanolide compounds are released during digestion.
- Small intestine (Absorbed): Absorbed into the bloodstream from the small intestine.
- Bloodstream (Circulates): Circulates through the blood before reaching hormone-regulating tissue.
- Stress hormone system (Modulates cortisol): Appears to modulate the body’s stress-hormone response with consistent use.
What the evidence supports
- Reducing self-reported stress and anxiety — moderate evidence. Several small randomised trials show consistent direction and reasonable effect sizes. Most ran 8 to 12 weeks, many were industry-funded, and sample sizes were small.
- Improving sleep quality — limited evidence. Some supportive trials, mostly in people with existing insomnia, and generally secondary outcomes rather than the main question.
- Increasing testosterone or muscle mass — limited evidence. A handful of small trials in men show modest changes. Not enough to treat as established.
- Evidence on long-term safety beyond about three months — insufficient evidence. Worth stating plainly: almost no trial ran long enough to answer this, and liver-injury reports have prompted regulatory review in several countries.

300–600 mg daily of a root extract standardised to withanolides
Upper limit: No established UL. Most trials ran 8–12 weeks, so long-term safety is genuinely unknown
Timing
Evening use is common for sleep-related benefit.
Time to effect
- 1–2 wks: Initial changes.
- 6–8 wks: Fuller stress-related effects, matching study durations.
Medication interactions
- Thyroid medication: Ashwagandha raises thyroid hormone levels and can push you into over-replacement. (high)
- Immunosuppressants: It stimulates immune activity, working against the drug’s purpose. (high)
- Sedatives, benzodiazepines, and alcohol: Additive sedation. (moderate)
- Diabetes and blood pressure medication: May lower blood sugar and blood pressure further. (moderate)
Common myth
“It works instantly like a sedative.” In reality: Most supporting research used daily dosing over several weeks, not a single dose.
