Stage by stage

What the evidence supports
- Slowing or partially reversing vision loss in Leber hereditary optic neuropathy (LHON) — moderate evidence. This is idebenone’s best-evidenced use. The randomized RHODOS trial and the larger LEROS study both found meaningful visual acuity benefits, particularly when started within a year of symptom onset, and idebenone (as Raxone) is approved for LHON in Europe, though not by the FDA.
- Improving cognition in Alzheimer’s disease or other dementias — insufficient evidence. Idebenone was originally developed and trialed for Alzheimer’s disease in the 1990s, but larger follow-up trials failed to show consistent cognitive benefit, and it isn’t used for this indication in current practice.
- Slowing progression of Duchenne muscular dystrophy — limited evidence. Idebenone received orphan drug status and was studied in phase 3 trials for Duchenne-related respiratory decline; the FDA rejected approval in 2017 citing insufficient evidence, though the EMA later approved it in Europe for a specific patient subgroup.
Typical dose
General antioxidant supplement products often suggest 45–150 mg/day, far below what clinical trials used for LHON
Upper limit: No established UL for over-the-counter use; the approved LHON drug dose is 900 mg/day under medical supervision
Timing
Take with a fat-containing meal; medical-grade LHON dosing is split across three daily doses.
Time to effect
- Months: In LHON trials, visual acuity changes emerged over months of continuous, high-dose treatment.
Common myth
"Idebenone is a proven general antioxidant and anti-aging supplement." In reality: Its strongest, trial-backed evidence is narrow — one inherited eye disease, at doses much higher than typical wellness products — not a validated broad anti-aging or energy benefit.
